The 2022 OC HMIS: Project Intake Forms in English, are available for use. They incorporate the recent changes to the Data Standards. In addition to the General/CoC Intake Form, separate forms are accessible for projects that receive HOPWA, PATH, RHY, SSVF, and VASH funding.
