New HMIS Intake, Exit, and Annual Assessment Forms!

As of October 1^st, the data elements in HMIS have been updated to reflect the new Data Standards released by HUD.  The HMIS Intake, Annual Assessment, and Exit forms have also been updated to match HMIS, and are published on our website here.  For more information, check out the October 2016 Data Matters Newsletter!
